1. When your in a heads up match do you want to almost raise everytime on the button?

you should be raising a lot hands on the button heads up.
average raise depends. in a 1-2NL game it can go from 12-25. online it's usually 6-8. in 5-5NL game, it can be $25 to 60. very rarely opening bets will go up to $65-100

1) Raise or fold. Unless, if you limp one or two times and they immediately go all in, sensing your weakness, use a limp to trap w/ a hand
2) How much? I prefer to mix things up a lot. Usually a 2-3 times the big blind is enough heads up.
A lot depends on what kind of game you are talking about. Is this the end of a tourney? Are blinds HUGE compared to chip stacks? Cash game?

1) If you plan on playing your hand, then it is a good idea. Just be careful. If you raise every hand expect him to re-raise when he gets something.
2) There is no standard. Every table you sit at will have its own typical raise. Some tables with be 3xBB other will be 6xBB. Most common would be about 4xBB

hjg, be careful of falling into the trap of acting in a standard way.
For one thing, it's all situational; depends on your position, who's acted before you and how, who's left to act and how they are likely to, among many other factors.
At least as importantly is to not fall into a pattern so you're harder to read.
