Before you start a thread about hand analysis, make sure to describe the situation completely. Left out facts like info on bet sizes, position, size of stack, size of pot, etc. just cause threads to be excessively long and unreadable. Nobody likes to piece together all the essential information tossed in throughout a series of posts.
So try and get all your info in the first post, por favor.
Some stuff that you should always list...
1. Game type - tourny or ring? NL, PL, or FL? Live or online?
2. Levels & Payout - In tournies, how far along are you? What's the payout like? In the money yet or on the bubble?
3. Blinds and Antes
4. Number of Players and your position in the hand
5. Position of all other key players in the hand
6. Your hand (duh) and your stack size
7. The stack size of those other key players in the hand
8. The size of bets
9. Your feel on the other key players in the hand, are they playing agro or tight? You think they're good or do they play like Geno? etc.
10. How you've been playing so far, and how what you think your image is at that table.
There's more, like describing how the action moves along, but you get the idea.

Yes - I think what you're wearing should always be factored in to the hand and how it was played. Ask yourself "Was I dressed aggressively enough to represent hand X?"

I'd add to not include results. Everytime someone includes their oppoent's holdings they get a bunch of partial results. It's always fun to know what the other guy had but it's counterproductive to include it at the beginning. Also if you have a turn question don't include what the river was.
That's what makes sense to me, but you're the site adm.

'Just for fun I tried the hand converter of an abominable enemy poker forum, and it worked nicely.
It works for partypoker, empire poker, paradise, absolute, pokerroom, pokerstars and ultimate bet. Using it is simply a matter of copying your hand history, pasting it into the window labelled 'input, click and chose 'FTR' from the format menu and click convert. Now copy the messy text that is displayed in the 'output' window and paste that into your message here on the forum.'

I saw a post recently where the poster did something really, really clever. They posted the hand (using one of the converters that deuces recommends--very nice) but they posted the hand results in white font (see the 'Font colour' setting when creating a post). The white font is unreadable, so doesn't give anything away, but the reader simply highlight it when you want to see it and there it is. Much cleaner than using lemon juice and a candle and it doesn't attract ants.

You would get an amazingly accurate "analysis" from people looking at the results THEN posting.
