I am thinking about building my own custom poker table and was wondering where you can get the cup holders that mount in the table top? or does anyone know where you can purchase the type used by casinos at their gaming tables?

Most casinos and/or cardrooms serve drinks and food on small tables that are placed behiind the player. Good and drink is not generally allowed on the table. That having been said, there are some cardrooms and casinos that do allow drinks on the table and they usually use a holder that slips under the rail. I have built several tables now and have found the cheapest cup holders on ebay-
I think the last set (of 10) was 6.00 + 6.00 shipping
