I was just wondering if there was a set buy-in to blinds ratio, and if so, what it is?
If not, what is recommended for A) casual players and B) tournament players?

To clarify, we mostly play with a $ buy-in and $0.01 small blind, $0.02 big blind, making our buy-in to blinds ratio about 33.

Cash/ring games, it's pretty standard that players buyin to the table with a max of 100 bb (2.00 for your .02 bb example).
Tourney play, the buyin should have nothing to do with the blinds.
