When you are down to two players, I have always played such that the dealer is the big blind (dealer, left of dealer is small blind (other player), left of small blind (dealer) is big blind). Though on TV yesterday, I saw that they had the dealer as the small blind. Which is it? Are there any other situations like this that I should know about?

You are correct. Once it gets to "heads up", the dealer is always the small blind. Why do you ask? If the dealer was the big blind, he would act last on every single round of betting. So to keep things somehwat fair, the dealer beecomes the small blind.
