How does the bet and raise go with a $1 small blind & $2 big blind?
Is it $1-$4 bet & $1-$4 raise before the flop?
$1-$4 bet & $1-$4 raise after the flop?
$1-$8 bet & $1-$8 raise after the turn?
$1-$8 bet & $1-$8 raise after the river?
Also, if someone opens or bets $4 after the turn or river, can another player throw out $12 making a $8 raise?

First off, if it's a 1-4-8-8 game, wouldn't the blinds be $.50/$1? A $2 blind doesn't make sense as it exceeds the minimum bet.
In a 1-4-8-8 spread game, you can bet anything from $1 to $4 before and after the flop, and anything up to $8 on the turn and the river.
And yes, if a player bets $4 on the turn or river, it can get raised to $12 since $8 is the maximum bet/raise.

That is the game I played in at the President Casino.
You would have to at least play (call big blind) $2 to see the flop.
